Requirements:

• Enrolled in health care college course

• Evidence of a “B” average in college course

• A completed application with a clearly though out, logical, and creative interpretation on the required essay on the application form.

• An official high school and college transcript from all schools attended

• Three completed reference forms:
  1) From a health care department faculty member
  2) From a former or recent employer,
  3) A personal reference

• References are required without exception
